This follow-up assessment will allow the provider to track symptoms, evaluate the effectiveness of the chosen treatment plan, and make any necessary adjustments. Follow-up assessments are completed; 1. After Initial Assessment 2. After hospital Discharge 3. After medication initiation 4. After medication adjustment
